If you know a child who might miss out on the happiness of a birthday celebration due to financial hardships or other challenges, our Birthday Box program is here to help.
Eligibility Criteria: To be eligible for a birthday box, applicants must meet the following criteria:
The child must be between the ages of 0 and 18.
The family must demonstrate financial need.
The application must be submitted at least one month before the child’s birthday to ensure timely delivery.
How to Apply:

Fill out the application form below. Please provide all requested details about the child, including their age, birthday, interests, and any special considerations (allergies, etc.).
Review Process:
Our team reviews applications on a rolling basis. Once your application is submitted, we will review it to ensure it meets our criteria and then contact you if any additional information is needed.
Notification:
You will receive an email, text, or phone call regarding the status of your application. If approved, we will discuss the logistics of delivering the Birthday Box to ensure it arrives in time for the child’s special day.
Privacy: Your privacy is important to us. All information provided will be treated with confidentiality and used solely for the purpose of assessing your application and delivering the birthday box.
Contact Us: Applications include up to 5 children for the household.
