Who Is Eligible to Request a Box?

• Child must live in Eagle Mountain, Saratoga Springs, Cedar Fort, Fairfield, UT or Primary Children’s Hospital - Miller Campus.

• Must demonstrate financial hardship.

• Child must be under 17 years old.

• Application must be submitted 30 days before box is needed.
